Sleeve, bypass or SADI-S in Iasi: a short guide

All bariatric operations here are laparoscopic, performed through four small punctures of 5 to 15 mm with cosmetic sutures, so the marks become almost invisible after healing. This greatly shortens recovery. For those from Iasi, the choice between sleeve, classic bypass, mini bypass and SADI-S depends on BMI, related diseases and the result you want to reach.

It begins with a surgeon's consultation: medical history, choice of method and a recovery plan. Patients from Iasi then complete a work-up — blood and urine tests, biochemistry, HbA1c, coagulation, chest X-ray, ECG, abdominal and kidney ultrasound, leg vein Doppler and a required gastroscopy — plus consultations with a GP, endocrinologist, psychiatrist, cardiologist and, for women, a gynecologist.

Weight loss is written into the contract, and the package includes every stage: consultation, diagnostics, the operation, inpatient care with meals and a year of support. A personal coordinator guides you from day one. To begin, patients from Iasi leave a request on the page or message the clinic, then receive a free consultation with a bariatric surgeon.

What the national insurance fund does and does not do for weight-loss operations

Romania's social health insurance is run by CNAS, the national authority that manages the system and a basic package of medical services, medicines, sanitary materials and medical devices. For someone in Iasi weighing up a bariatric operation, the first practical question is whether this package pays for it. The answer, based on a Romanian news article of 26 December 2024, is that the fund does not fully reimburse bariatric surgery, in either state or private hospitals. The same article gives no BMI criteria, so it would be wrong to promise that a given body mass index opens the door to funding.

The article also describes a middle path. Some preoperative and postoperative services, endoscopy among them, can be free if you have a referral from a family doctor. That matters because the preparation for this kind of operation includes several examinations. The programme described to us includes blood and urine tests, an ECG, an ultrasound and a gastroscopy, and appointments with specialists. The exact list is given by the doctor during the online consultation. It is worth asking your family doctor what can be arranged through the insurance system and bringing the results to the conversation.

Free procedures in Romania generally require a health card showing that insurance has been paid. EU citizens and uninsured Romanians receive free emergency care, but emergency care is a different matter from planned obesity treatment. Since April 2019, insured people may also pay the difference between a private provider's tariffs and the fund's fees. In practice, how a particular examination is handled depends on the provider and on the referral, so confirm it before you book anything.

Our coordinators do not replace the insurance office or your family doctor. What they can do is explain the medical side clearly, so that you arrive at those conversations knowing which tests are usually needed. Write to us on WhatsApp or Telegram at any hour; the first consultation is free, and it does not commit you to an operation.

A hospital-centred system, rising co-payments and why people compare options

Romania's health system is described as hospital-centric, with a shortage of healthcare workers. We found no official figure for waiting lists for weight-loss operations, so we will not quote one. What we can say is that a person who is told to wait, or who simply wants a clear plan sooner, often looks for an additional opinion. An online consultation is a way to get one without leaving home, and it takes the form of a conversation and a few photos rather than a queue.

The financial context is also documented. Out-of-pocket payments were about 21% of total health expenditure, against an EU average of 15%. Those figures come from dated data, so treat them as a general picture rather than today's measurement. CNAS has also begun introducing co-payments for certain services that are not strictly medically necessary. For patients, the lesson is simple: ask in advance what is covered, what is not and what is written into a contract, and keep every document.

Our approach to this is plain. After the consultation, the doctor names the final amount, and it is fixed in the contract before anything is booked. No operation is scheduled until you have confirmed the plan and the dates. If you decide that it is not the right moment, you have lost nothing but a conversation.

It is also worth being honest about the limits of a remote assessment. The doctor can discuss your history, your BMI and your goals, and can look at the photos you send. The final choice of method still depends on the tests done before the operation and on your anatomy and general health.

Who is a candidate: the BMI thresholds and the exclusions

Bariatric surgery is the medical term for weight-loss surgery, and it is a treatment for obesity and related illnesses rather than a cosmetic procedure. It changes how the digestive tract works. Operations are generally justified at a BMI above 40, or above 35 when serious related conditions are present. Sometimes lower values are considered when non-surgical methods have not worked. BMI is body weight in kilograms divided by height in metres squared, so you can calculate your own value before writing to us.

Conditions that surgery may help with include type 2 diabetes, hypertension, sleep apnea, joint disease, fatty liver disease, varicose veins and some reproductive health problems. The two core methods are sleeve gastrectomy and gastric bypass. The sleeve reduces stomach volume and is suited to obesity without marked reflux. Bypass reduces volume and changes the path of food, and it is considered for obesity with diabetes or reflux. The doctor chooses between them on the basis of your BMI and health.

Some people are not suitable. The exclusions are age under 18, malignant tumours, pregnancy, blood clotting disorders, severe chronic and mental illness, an acute illness or flare-up of a chronic disease, and less than six months since breastfeeding ended. If you are unsure which column describes you, the coordinator will tell you in the first conversation whether it is worth going further.

Because the operation is carried out laparoscopically, through 4 small punctures of 5 to 15 mm, with cosmetic sutures, the marks usually become almost invisible once healed. The hospital stay is from 1 to 5 days under observation, depending on how complex the operation was and how you feel.

Planning recovery around Iasi's winters, holidays and calendar

Recovery has a rhythm that you can set against the calendar. The programme says that office work is possible after 2–4 weeks, physical work after about 1.5 months, and swimming after 30 days. Other exercise is added gradually as weight drops. These are averages, and the doctor confirms them for you. Placing them next to the local year helps you to choose a start date that does not clash with work.

Iasi's climate is a practical consideration. Long-term averages for 1991–2020 show a January mean high of 1.3 °C and a low of -5.1 °C, with February snow of 14.3 cm in the older 1961–1990 normals. December is the most humid month at 83% and gets only 61.5 hours of sunshine. July is warm, with a mean high of 29.0 °C. Whether you prefer to recover in the dark, cold weeks or in summer is a personal decision, but if you expect icy pavements and short days, plan a quiet first fortnight at home and avoid errands on slippery ground.

Public holidays are the other part of the calendar. Romania has 15 non-working days, among them 1–2 January, 24 January, Orthodox Easter from Friday to Monday, 1 May, 1 June, Pentecost, 15 August, 30 November, 1 December and 25–26 December. Orthodox Easter Sunday falls on 2 May 2027. A holiday block can be a convenient way to cover the first days after discharge, but a return to the office should still follow how you feel rather than the date on the calendar.

Remember that follow-up does not depend on opening hours. Coordinators answer round the clock, 24/7, so a question about a symptom, a meal or a test result on a public holiday or a snowy evening can be put in writing at once, and the team replies.

Changing the plate: staged eating, vitamins and a year of remote follow-up

After the operation, eating changes in stages, from liquid to soft to solid food. The main emphasis is on protein and on restricting high-calorie, fatty food. Our source material has no data on Romanian dishes or on Moldavian food habits, so we will not claim that a particular local dish is suitable or unsuitable. What a coordinator can do is take the foods you actually eat at home and ask the doctor how to fit them into the staged plan.

Vitamins are essential after bariatric surgery. Because the anatomy of digestion changes, the body absorbs certain nutrients less well, and regular vitamin and mineral supplements become a lasting part of life. Weight regain is possible, but it is very unlikely if you follow the doctor's advice. The advice is simple to state: eat in the way you were taught, avoid high-calorie drinks and keep attending follow-up.

The package includes support for 12 months, with one coordinator from the first day and a remote progress check by photos. After discharge you receive guidance on diet and recovery, and scheduled check-ups of your well-being, weight and test results. Because it is online, none of this requires you to be in a waiting room, and it can be done from the kitchen table while you cook.

The final result comes 18–24 months after the operation. That is a long time, which is why we describe the process as a relationship and not a single event. If you plan a pregnancy, doctors advise waiting at least 12–18 months, a period in which the body stabilises, weight reaches target values and nutritional status normalises.

A student city and a pilgrimage season: fitting online care into a busy calendar

Iasi has over 60,000 students in five public universities. Alexandru Ioan Cuza University alone had 24,047 students in 2019–2020 across 15 faculties. We cannot say how many of them struggle with weight, and we do not want to guess. But the figures do tell you something useful: a lot of the city's population is accustomed to studying, applying and consulting by message. For them an online consultation is a natural format, and the contraindication of age under 18 means the service is for adults.

The city's year also has loud weeks. The Saint Parascheva pilgrimage around 14 October draws hundreds of thousands of pilgrims and coincides with the city's Celebration Days. Festivals are held through the year: Hangariada each May, the International Theatre Festival for Young Audience each October, FILIT for literature since 2013 and Afterhills for music since 2017. In busy weeks it is easier to arrange messages and photo checks than to move across a crowded city, and the coordinators are available at any hour.

The city is not short of ways to stay active. CTP Iasi runs 126 trams and 150 buses, and electric trams have operated since 1900. FC Politehnica Iasi plays at the Emil Alexandrescu Stadium, and CS Politehnica Iasi has basketball and rugby sections. Walking to a tram stop or taking a short route on foot can become part of a gradual return to activity, as the doctor allows. The figure of 50,358,000 passengers in 2014 gives an idea of how widely public transport is used.

If you live in a hilly part of Iasi, such as around Copou, Cetatuia or Bucium, gradual walking will feel different from walking on flat ground, so mention it during the consultation. The city sits on the Bahlui river at an elevation of about 60 m, with hills among its famous 'seven hills'. Tell the coordinator about your daily routes, and the doctor can advise how quickly to increase them.

Does the Romanian insurance fund pay for a gastric sleeve or bypass?

According to a Romanian news article of 26 December 2024, CNAS does not fully reimburse bariatric surgery, in either state or private hospitals. Some pre- and postoperative services, such as endoscopy, can be free with a family-doctor referral. The article gives no BMI criteria, so ask your family doctor and your insurance office what applies to you.

Can I use my family doctor's referral if I consult online?

A referral can help you obtain some preparatory examinations, such as endoscopy, through the insurance system. The online consultation does not depend on it, and you can start with a free conversation and photos. Bring any results you already have, because the doctor can use them to plan the list of tests.

Which tests will be needed before the operation?

The programme includes blood and urine tests, an ECG, an ultrasound and a gastroscopy, plus appointments with specialists. The doctor gives you the exact list at the consultation. Ask your family doctor which of them can be arranged through the insurance system.

Is a winter date a problem for recovery in Iasi?

Winters are cold: the January mean low is -5.1 °C and the mean high is 1.3 °C, and December has 83% humidity. Recovery does not require warm weather, but plan a quiet first fortnight at home and avoid icy ground. Office work is possible after 2–4 weeks, so a winter date works if your routine can be arranged.

Can I schedule the operation around a public holiday such as Orthodox Easter?

Yes, many people use a holiday block to cover the first days. Orthodox Easter Sunday falls on 2 May 2027, with the Friday to Monday around it non-working. The hospital stay is 1–5 days, and return to the office is 2–4 weeks, so the doctor will help you to choose dates that suit both medicine and work.

I live in a hilly part of the city. When can I walk and exercise?

The programme says that swimming is possible after 30 days and that other exercise is added gradually as weight drops. Physical work comes after about 1.5 months. Describe your daily routes to the coordinator, and the doctor will tell you how to build up.

Can I get follow-up support if I am busy with studies or work?

Yes. Support lasts for 12 months, with a coordinator from the first day and a remote progress check by photos. Coordinators reply round the clock on WhatsApp and Telegram, so you can write between lectures or after work. Scheduled check-ups cover your well-being, weight and test results.

Which local dishes will I be able to eat afterwards?

We have no sourced data on Romanian food, so we do not give a list. After the operation the diet moves from liquid to soft to solid, with the focus on protein and the restriction of high-calorie, fatty food. Tell the coordinator what you normally cook, and the doctor will advise how to adapt it.
