Comparison

Liposuction techniques

We use two liposuction techniques: tumescent (classic) and VASER (ultrasound). Compare the goal, the indications and the anaesthesia to choose the right technique together with your surgeon.

Tumescent liposuction

A special solution loosens fat for removal through a thin cannula

Tumescent liposuction: abdomen and thighs before and after surgery

from $1,200

Main goal
Removal of local fat with minimal blood loss
When it is done
Abdomen, flanks, thighs, back, arms, chin
Anaesthesia and stay
Local or general anaesthesia, home the same day or after 1 day

VASER liposuction

Ultrasound breaks down fat cells before vacuum removal

VASER liposuction: a male patient before and after surgery

Price at consultation

Main goal
Defined muscle relief and body contouring
When it is done
Athletic abs and delicate areas
Anaesthesia and stay
Local or general anaesthesia, 1–4 hours of surgery

The benefits and recovery times shown are averaged for each technique and given for guidance: the scope of correction and rehabilitation depend on your anatomy and individual features, and the surgeon confirms them at the consultation.

At the consultation the surgeon draws up a plan for your case and explains how your liposuction will go.

Free, before surgery and with no obligation.

Recovering after liposuction

After tumescent liposuction, recovery is much shorter than after a conventional procedure. Most patients return to work within 7–10 days. In the first week, mild swelling and a raised temperature are possible — a normal response to surgery. Compression garments are worn for 30–45 days: they fix the new contours and speed up tissue recovery. In the first month, bathhouse, sauna, pool and intense sport are off-limits. For the best result and faster recovery we recommend lymphatic drainage massage, pressotherapy and a gradual return to activity. First visible changes show 2 weeks after surgery, when the main swelling subsides; the final result forms in 3–6 months.

Post-op check-ups

After 7 days, at 3 months and at 6 months. Until the end of the year, get a post-surgery consultation with your surgeon to review results and get more information, at the clinic's expense.

A compact city between hills and a river

Bilbao covers roughly 40.65 km² and has an official mean altitude of 19 m, with elevations between 6 and 32 m. The built-up area sits roughly 16 km south of the Bay of Biscay and is surrounded by two modest mountain ranges of around 400 m. The highest point is Mount Ganeta at 689 m. The Nervión and the Ibaizábal meet in Basauri and form the Bilbao estuary, about 15 km long, with an average flow of around 25 m³/s. Its main tributary is the Cadagua. The municipality had 347,342 inhabitants in the 2024 INE count, and the metropolitan area has about 1,037,847.

These numbers matter for body contouring because daily movement here is shaped by the terrain. The centre is low and flat along the river. The ring around it rises to about 400 m. Someone who walks, cycles or runs along the estuary has a very different load from someone who climbs toward the surrounding ridges. We bring this up in the first conversation because the doctor needs to know how you move before planning anything. Regular activity on level ground and a habit of hill walking both fit with liposuction, but the return to them is staged differently.

Liposuction is meant for local fat that does not respond to diet and exercise. Typical areas are the abdomen and waist, outer and inner thighs, the back including the lower back, the arms, the chin and neck, the knees, and in men the fat component of the chest. If you already have a stable routine, the doctor looks at where the fat stays despite it. For example, an active person may have a flat, strong core and still have a soft pocket on the flanks. That is a realistic case for the procedure. A person whose main aim is to reduce overall body weight is a different case, which we cover below.

You send photos on WhatsApp or Telegram, the doctor assesses them, and you get a free online consultation with an answer from the coordinators round the clock. Nothing is booked at that stage. You describe your activity, whether it is walking in the centre, cycling along the estuary or gym work, and the doctor explains when each can return.

Mild summers, wet winters and what a compression garment means in each

Bilbao has an oceanic climate (Cfb). Using the 1991–2020 normals, August and July are the warmest months, with average highs of 26.3 and 25.4 °C and lows of 16.1 and 15.8 °C. January and February are the coolest, with highs of 13.5 and 14.2 °C and lows of 5.3 and 5.1 °C. The record maximum is 44.0 °C and the record minimum −8.6 °C. Extremes like these are rare, but they show that the usual range can be exceeded.

The compression garment is worn for 30–45 days after surgery. It is fitted on the day of the operation, and its job is to fix the new contours and support tissue recovery. Its comfort depends on the season in which that period falls.

If the 30–45 days fall in winter, with January highs of 13.5 °C, the garment sits comfortably under normal clothing. The winter months are also those with the least sun, only 81 hours each in December and January. For someone who dislikes beach clothing and prefers layers, this is a convenient time. The other side is that rain is frequent. Annual precipitation is 1,158 mm over 124.4 days, with November the wettest month (171 mm over 13.7 days). Fine drizzle, known locally as sirimiri, adds to the damp. Humidity averages 71 %. A damp garment is uncomfortable, so a spare set and a plan for drying matter. The doctor and coordinator explain how to care for the garment during the consultation.

If the period falls in July or August, the garment is worn under warmer conditions. The mean highs of 25.4 and 26.3 °C are not extreme, but a close-fitting garment feels warmer than ordinary clothing. Direct sun is also a concern: July has 186 hours of sun, and the first weeks are not the time for tanning beds or long exposure. The service rules also exclude the bathhouse, sauna, pool and intense sport in the first month, which affects how you plan a summer. Swimming in the sea at the coast is also part of that month's restriction on pools, so ask the doctor about water in general.

Neither season is better in absolute terms. Winter is easier on the garment, and summer is easier for walking outdoors. The right choice is the period in which you can reliably follow the recovery rules, so we ask about your calendar during the consultation.

Also consider that the final result forms in 3–6 months. If you want the contour to be settled before the warmest months, count back from that time. The first visible changes appear at 2 weeks, once the main swelling subsides. Swelling and bruising fade by weeks 4–5, and an interim result is visible at 1.5–2 months.

Weight, fat and the national figures

The 2020 European Health Survey, published by the INE, found obesity in 16.0 % of the Spanish population aged 18 and over (16.5 % of men and 15.5 % of women). Overweight was recorded in 37.6 %. In total, 53.6 % were above what is considered normal weight. These are national numbers, not figures for Bilbao, and we do not claim that they describe the city.

They are useful here because they frame what liposuction is for. It is not a weight-loss method. It removes local fat deposits that resist diet and exercise and reshapes the contour. A person with a high overall body weight is usually better served by a different route first. Our doctors say so openly: if your case can be solved without surgery, the doctor tells you at the consultation. If your weight is unstable, the first step is a nutritionist or endocrinologist.

The list of conditions that make liposuction unsuitable includes endocrine obesity, decompensated diabetes, blood coagulation disorders, serious heart, kidney or liver disease, cancer, pregnancy and the breastfeeding period, and age under 18. Before the operation an endocrinologist rules out hormonal causes of the fat deposits, and a hormonal screening is part of the plan. This helps predict the long-term result.

There is also a distinction between cosmetic contouring and obesity treatment. In the common portfolio of the Spanish national health system, Royal Decree 1030/2006 includes the adjustable gastric band for adults with a BMI above 40 kg/m², or above 35 with significant associated comorbidity, after conservative alternatives such as diet, exercise and behaviour change have failed. That is a different medical pathway from liposuction. If your question is really about severe obesity, the consultation will tell you honestly that liposuction is not the answer, and bariatric options are a separate conversation.

On weight stability, the practical message is simple. Removed fat cells do not come back, but the remaining ones can grow if you gain weight. A strict diet is not required after the operation, but a balanced diet and regular physical activity help keep the result. Many people ask whether to lose weight first. The doctor will tell you whether your weight has been stable enough to plan, which is why we ask for your weight history in the first messages.

The same survey places osteoarthritis as the third most common chronic diagnosed problem in people aged 15 and over (14.4 %), after hypertension (19.0 %) and high cholesterol (15.3 %), with low back pain at 13.7 %. These are national numbers, but they point to something we ask about: joint and back complaints affect how you can walk and exercise after the operation, and they matter when the lower back or thighs are treated. Tell the doctor about them.

Why most people in Spain plan this privately, and what the waiting lists show

Spain's national health system (SNS) has a common portfolio of services defined in Royal Decree 1030/2006. Article 5.4.a.4º says that techniques whose aim is "mere aesthetic or cosmetic improvement" are not financed. Procedures with an aesthetic purpose that have no relation to an accident, illness or congenital malformation are excluded from specialised care. In plain words, cosmetic surgery is outside the public system. Liposuction for contouring falls into that category, so a person in Bilbao who wants it is normally planning a private procedure.

The same decree treats implants in a similar way. Breast implants and skin expanders are included as reparative implants, but not when they are used in cosmetic surgery unrelated to accident, illness or congenital malformation. The principle is the same: the purpose of the intervention decides.

The waiting-list data show why it is useful to know how the system works, even if you never use it for this purpose. On 31 December 2025, the SNS had 853,509 patients on surgical waiting lists, with an average wait of 121 days, and 21.6 % had waited more than 6 months. A later note corrected the figures for Castilla-La Mancha, so the initial numbers may be slightly revised. Plastic surgery was the specialty with the longest average wait: 269 days. Neurosurgery was at 172 days. For hip prostheses the average was 98 days, with 13.7 % beyond the maximum period of 180 days. For knee prostheses it was 105 days, with 15.9 % beyond it.

The Basque Country has one of the shortest waits in the SNS. On the same date there were 24,069 patients in structural surgical waiting (10.81 per 1,000 inhabitants), with an average wait of 64 days. Traumatology accounted for 6,638 patients with an average of 78 days. For first specialist consultations nationally, 84.42 of every 1,000 people were on a waiting list, with an average of 102 days, and Traumatology had the longest consultation wait at 132 days.

We do not use these numbers to criticise the public system. They explain a practical point: a contouring procedure is arranged privately, with a schedule agreed with you in advance, and the final price is fixed in the contract before surgery. Public services remain available for everything else, including your routine care and any follow-up you need with your own physician. If you have a health condition that could affect the procedure, such as joint problems, the doctor will ask you to share the relevant medical records, and your regular physician stays involved.

Fitting recovery into the calendar: Aste Nagusia, the university year and a worked example

Bilbao's calendar has fixed points that affect planning. Aste Nagusia, the Semana Grande, lasts nine days, starting on the first Saturday after 15 August, and opens with the chupinazo from the Teatro Arriaga. The University of the Basque Country (UPV/EHU) has its Bizkaia campus in Leioa, 11 km from the centre, and the university's roots go back to the University of Bilbao of 1968. Students and staff follow an academic year, and workers follow their own holiday periods.

We do not claim anything about how people spend these days. We only point out that a fixed event can be a deadline you set for yourself, and a deadline needs a margin. Because the final result forms in 3–6 months, a person who wants to be at ease with their body at a late-August event should plan the operation well before it, not in the weeks before. The same applies to the start of a semester or the return from a holiday period.

A worked example makes the sequence concrete. Suppose a consultation takes place in December 2026 and the operation is on 11 January 2027.

- On the day of surgery, the compression garment is fitted. Under local anaesthesia you go home 3–4 hours after the operation. Under general anaesthesia you stay one day in a comfortable ward.

- At 7 days, around 18 January 2027, is the first check-up.

- Most patients return to work within 7–10 days, so roughly 18–21 January.

- The first visible changes appear at about 2 weeks, around 25 January, when the main swelling subsides.

- Swelling and bruising fade by weeks 4–5, in the middle of February.

- The compression garment is worn for 30–45 days, so until about 10–25 February.

- The interim result is visible at 1.5–2 months, in late February to early March.

- The 3-month check-up falls around 11 April, and the 6-month check-up around 11 July 2027.

- The final result forms between April and July 2027.

The dates are an illustration of the arithmetic, not a promise for your case. Your own plan depends on your anatomy, the scope of the operation and the technique. A small area takes about 1 hour, and a combined procedure takes 3–4 hours. Overall, the duration is 1–4 hours.

In the first week, skip physical exertion and heavy lifting. For 4–6 weeks, wear the garment and avoid the bathhouse, sauna, pool and active sport. Keep away from alcohol and tanning beds for at least a month. Before the operation, give up smoking, alcohol and medications that affect clotting, such as aspirin, 7–10 days in advance. The doctor gives you the full list after the first consultation.

What the online consultation covers, step by step

You live in Bilbao and the first consultation is online, so the whole preparation can start without leaving home. You send photos on WhatsApp or Telegram. The doctor assesses them, comments on the fat deposits and skin tone, names the technique that fits and gives the final price, which is then fixed in the contract. The coordinators answer round the clock, and the reply to a first message normally comes within 15 minutes.

There are two techniques. Tumescent (classic) liposuction uses a special solution that loosens the fat so it can be removed through a thin cannula, with minimal blood loss. VASER uses ultrasound to break down fat cells before vacuum removal and is chosen for defined muscle relief and body contouring, such as athletic abs and delicate areas. Both can be done under local or general anaesthesia. The fat is removed through micro-punctures of 3–5 mm, which usually need no stitches.

The skin matters as much as the fat. Skin tone, elasticity and turgor are checked before the operation. Good skin tightens on its own and adapts to the new contours. If the tone is reduced or stretch marks are present, the specialists recommend preparatory treatments in advance. If the main issue is excess skin rather than fat, the doctor may suggest something else, such as a tummy tuck, and will explain why.

The steps after the consultation are:

1. The doctor draws up a plan and rules out contraindications.

2. A date is reserved, and the terms are fixed in the contract for medical services.

3. Pre-operative tests are done: blood count, coagulation panel, biochemistry, infections, an ECG, a chest X-ray and, if needed, a GP consultation.

4. On the day, the areas are marked and the garment is fitted.

5. You wake up and are monitored, then go home or stay one night.

6. Recovery follows, with progress monitored remotely by photos and check-ups at 7 days, 3 months and 6 months.

The price includes the scheduled dressings, the post-operative check-ups, a personal coordinator on call, the compression garments fitted on the day of surgery, a stay in a comfortable hospital ward with meals, individual recovery recommendations and remote monitoring by photos. With general anaesthesia, recovery starts in the ward under round-the-clock nursing care.

If you are not sure that liposuction is right for you, leave your number and a coordinator will call you back. No booking is needed to find out.

Does the damp Bilbao winter make recovery harder?

It changes the practicalities more than the medicine. The city has an oceanic climate, with 1,158 mm of precipitation a year over 124.4 days, and November is the wettest month at 171 mm. Humidity averages 71 %. The compression garment is worn for 30–45 days, so a damp period means keeping a spare garment and letting each one dry fully. Winter highs of 13.5 °C in January are comfortable for a close-fitting garment under normal clothing. The doctor and coordinator explain garment care during the consultation.

Is it better to have liposuction in winter so the garment is not worn in summer heat?

Neither season is better in every case. July and August have mean highs of 25.4 and 26.3 °C, which is mild by many standards, but a close garment feels warmer than ordinary clothing, and tanning beds and long sun exposure should be avoided for at least a month. In winter the garment is easier to wear, but there are fewer hours of sun and more rain. The deciding factor is when you can follow the recovery rules reliably, including no pool, sauna or intense sport in the first month. We discuss your calendar and work backwards from the 3–6 months needed for the final result.

Will the hilly terrain around the city affect when I can walk and cycle again?

The terrain is a good reason to describe your activity in detail. Central Bilbao is low, with a mean altitude of 19 m, while the surrounding ranges reach about 400 m and Mount Ganeta 689 m. In the first 7 days, you skip physical exertion and heavy lifting. After that, gentle walking on level ground is the natural first step, and a gradual return to activity is recommended. Intense sport and hill climbing belong to later weeks, and the doctor tells you when each is appropriate in your case.

Does the national obesity data mean I should lose weight before liposuction?

Not automatically. The 2020 INE survey found obesity in 16.0 % of Spanish adults and overweight in 37.6 %, but these are national figures and not a rule for individuals. Liposuction is not a weight-loss method, and it is meant for local fat that resists diet and exercise. If your weight is unstable, the first step is a nutritionist or endocrinologist. The doctor assesses your weight history in the consultation and tells you whether to wait.

Can public healthcare in the Basque Country cover liposuction?

Generally no, when the purpose is contouring. Royal Decree 1030/2006 excludes techniques aimed at "mere aesthetic or cosmetic improvement" from the common portfolio of the national health system. Procedures with an aesthetic purpose unrelated to accident, illness or congenital malformation are outside specialised care. A contouring procedure is therefore arranged privately, and the final price is fixed in your contract before surgery. Ask your own physician about any coverage questions that relate to a medical condition.

Why do you mention waiting lists if this is a private procedure?

Because they explain how the public system is organised. On 31 December 2025, Plastic Surgery had the longest average wait in the SNS at 269 days, while the Basque Country had an average surgical wait of 64 days, one of the shortest. Those lists concern procedures the public system covers. Contouring is not among them, so you are not queuing behind anyone. The date is agreed with you after the consultation.

I am a student or work around the academic year. When should I schedule it?

Think in stages. Most patients return to work within 7–10 days, though the garment stays on for 30–45 days and the first month excludes intense sport. The final result forms in 3–6 months, so a quieter period before an important date is better than the last weeks before it. The University of the Basque Country has its Bizkaia campus in Leioa, 11 km from the centre, so students should consider the commute during the first week of recovery. The coordinator helps you build a calendar around exams or holidays.

I want to be ready for Aste Nagusia in late August. How far ahead should I plan?

Aste Nagusia runs for nine days from the first Saturday after 15 August. Because the interim result is visible at 1.5–2 months and the final contours at 3–6 months, plan the operation several months before the festival, not in the final weeks. Remember that the first month excludes the bathhouse, sauna, pool and intense sport. Send your photos early, and the coordinator can count back from your target date and propose a schedule.

How do I prepare my consultation from Bilbao if I have back or joint complaints?

Mention them in your first messages. Back pain and osteoarthritis are common chronic problems in Spain, according to the 2020 INE survey (osteoarthritis 14.4 %, low back pain 13.7 %), and they can affect how comfortably you move in the first weeks. Send recent photos in good light, note your weight history, medications and any earlier operations, and have your medical records ready.
