Bariatric surgery in Bologna: what you should know

SADI-S combines a sleeve with intestinal bypass in two stages and is one of the most powerful bariatric operations, with 80–90% excess weight loss and a long-lasting result. It suits super obesity with a BMI over 50, severe type 2 diabetes, or a second stage after an ineffective sleeve. Anyone in Bologna in these situations can discuss it at a consultation.

Step one: consultation. Step two: laboratory and instrumental tests, gastroscopy and specialist reviews. Step three: surgery through small punctures by a permanent team, not a duty shift. Step four: 1–5 days in the ward with meals and nursing. Step five: a staged diet and a year of follow-up. People in Bologna follow this route from start to finish.

Weight loss is written into the contract, and the package includes every stage: consultation, diagnostics, the operation, inpatient care with meals and a year of support. A personal coordinator guides you from day one. To begin, patients from Bologna leave a request on the page or message the clinic, then receive a free consultation with a bariatric surgeon.

What the national figures say about excess weight in Italy

The most recent ISTAT figures cover adults aged 18 and over in 2023. They put 46.3% of that group as overweight or obese: 34.6% overweight and 11.8% living with obesity. The obesity share was 10.3% in 2013. Over the same period, then, the proportion of adults with obesity rose by 1.5 percentage points. The same release reports that 35% of people aged 3 and over are sedentary. These are national numbers. The data we have does not split them by region, so we do not attach them to Emilia-Romagna.

Bologna is the capital of Emilia-Romagna and has 391,473 residents according to the 2026 infobox figure. The Italian-language article gives 392,052 residents at 31 May 2026. The Metropolitan City has 1,024,290 people, and the municipality covers 140.86 km2 with a density of 2,779.2 per km2. These figures matter for planning rather than diagnosis. A large urban area means many people are looking at the same questions about weight, public care and private options.

Excess weight is a medical matter before it is a matter of appearance. Our bariatric programme lists conditions that surgery can help with: hypertension, type 2 diabetes, sleep apnea, varicose veins, joint disease, fatty liver disease and reproductive health problems. If your doctor or your family have already linked your weight to one of these, the conversation with us starts from that list. It does not start from a number on the scales.

A first online consultation is free and does not commit you to anything. You send photos and basic details through WhatsApp or Telegram, and a doctor assesses them. Your BMI is calculated by dividing body weight in kilograms by height in metres squared. In general, surgery is considered at a BMI above 40, or above 35 when serious related conditions are present. Sometimes it is considered at lower values, when non-surgical methods have not worked. The doctor confirms during the consultation whether surgery suits you at all.

How Italy's public system treats bariatric procedures, and how that differs from a private route

Italy's national health service, the SSN (Servizio Sanitario Nazionale), was established in 1978. It gives universal coverage to citizens and residents and is run on a regional basis under the Ministry of Health. The Ministry sets the national Essential Levels of Care, known as LEA. In practice this means the region, here Emilia-Romagna, organises how public care is delivered to people in Bologna.

A secondary source from January 2024 draws a useful line. Purely aesthetic surgery is not paid for by the SSN and is available only through private services. By contrast, procedures needed to reduce the risks of severe obesity are described as reimbursable, "compresa la chirurgia bariatrica". The same article includes post-obesity abdominoplasty in that group. Bariatric surgery for medical reasons therefore sits on the covered side of the line. Treatment that only reshapes the body does not.

This is also the difference between bariatric and plastic surgery as we describe it. Bariatric surgery treats obesity and related illness by changing how the digestive tract works. Plastic surgery reshapes the outer contours of the body and does not treat obesity or affect metabolism.

The Italian-language article on bariatric surgery lists the criteria used in the country. They are:

- the patient is an adult;

- BMI is 40 kg/m2 or higher, or between 35 and 39.9 with one or more obesity-related diseases;

- conservative therapy has failed.

These criteria are close to the ones our doctors apply, as described above. If you already have a hospital assessment, send us the conclusion. It helps the doctor prepare for the consultation.

Waiting is the practical issue. English-language reference material on Italian healthcare describes waits as usually up to a few months in the big public facilities. Private facilities contracted with the SSN usually take up to a few weeks. We found no official waiting-list figure, so treat these as general descriptions only. The same source says patients sometimes move from poorer southern regions to the more prosperous north for care. That is one reason northern hospitals can be busy.

Co-pays, called tickets, apply to specialist visits without diagnostics. They are waived for low-income patients, and there are exemptions by age (over 65, under 6), income or chronic illness. If you plan to use the public system for tests, check your exemption status.

What we offer is a private route that you can use alongside any public assessment or instead of it. The first doctor assessment and price discussion cost nothing. Nothing is booked before you confirm the plan and the dates. The final price is set in a written contract after the consultation.

Fitting the diet stages into a Bologna week

After bariatric surgery, food changes in stages. The diet moves from liquid to soft to solid. The main focus is protein, with high-calorie and fatty food restricted. We have no sourced facts about Bologna's cuisine, so we will not describe it. What we can do is look at the rhythm of the city and ask where meals fall in it.

The University of Bologna, founded around 1088, reports 90,291 students in its infobox, with no year given. It has campuses in Cesena, Forlì, Ravenna and Rimini. The city article says over 80,000 students were enrolled in 2015. The university has 11 schools, 33 departments and 93 libraries. If you study or teach, your timetable is probably built around lectures and library hours. The liquid and soft stages take planning. Plan which meals you can prepare ahead and carry.

The city's public transport facts point the same way. The average commute is 5.4 km and takes 53 minutes per weekday trip. TPER has run trolleybus lines since 2012, and a first tramway line was under construction in May 2023, with four lines planned. A commute of that length means some meals fall away from home. Ask your coordinator to help you build a simple plan for the first weeks. It should cover which meals you eat where, what you pack, and what you can have ready in the evening.

Movement is part of the plan too. Swimming is fine after 30 days. Other exercise gets added gradually as weight drops. If you work physically, expect to return after 1.5 months. Office work usually resumes after 2–4 weeks. Given the national figure on sedentary behaviour, a gradual, written activity plan is useful.

Festivals and match days are also part of a Bologna calendar. The city lists Bologna Jazz Festival, Biografilm, Il Cinema Ritrovato, Gender Bender, Future Film Festival and Human Rights Film Festival. Bologna FC 1909 plays at the Stadio Renato Dall'Ara, and Virtus Bologna has 17 Italian basketball titles. We cannot say which dates will be busy, because the months are not given. We can say that social events often involve food. Tell your coordinator about events you do not want to miss. They can help you decide how to handle them in the first weeks.

Bologna's seasons and choosing a date

Bologna has a humid subtropical climate (Cfa). It sits on the edge of the Po Plain at the foot of the Apennines, where the Reno and Savena valleys meet. The 1991–2020 normals for July show a mean high of 31.4 °C and a low of 19.1 °C. August is almost the same, at 31.3 °C and 19.0 °C. January has a mean high of 7.2 °C and a low of -0.5 °C. Record extremes for the period are 40 °C in July and August and -17 °C in February and December.

Annual precipitation is 667.6 mm over 73 days with at least 1 mm. November (77.2 mm) and October (75.4 mm) are the wettest months. January (34 mm) and July (32.1 mm) are the driest. Annual relative humidity is 69.7%. It ranges from 79.2% in January to 59.1% in July, and reaches 81% in November.

What does this mean for recovery? Your hospital stay lasts 1–5 days under observation. After discharge, most people go back to office work after 2–4 weeks. Seasons matter mostly for comfort:

- In July and August, high temperatures make long walks harder in the first weeks, and you need to keep hydrated.

- In late autumn, the humid and rainy weather can make short outdoor walks less appealing.

- In winter, snow is rare. The 1961–1990 normals give 6.35 snowy days a year, with 2.4 in January. Cold mornings still affect how easy it is to get out and walk.

None of this decides the date by itself. Your health and the doctor's plan come first. Seasons are a secondary factor, and your coordinator can help weigh them.

Public holidays also matter, because they affect when you can arrange tests. The national list includes 1 November, 8 December, 25 and 26 December, 1 January and 6 January. Every Sunday is a holiday too. Clinics and laboratories may keep different hours around these days. Our coordinators answer 24/7 on WhatsApp and Telegram, so you can ask about tests or dates at any time, including holidays.

Before the procedure: tests, method and what is included

The treatment plan starts with a consultation, where the doctor assesses your health and discusses your goals. The doctor orders the tests needed to select the appropriate method. Before the procedure you have blood and urine tests, an ECG, an ultrasound and a gastroscopy, and see specialists. The doctor gives you the exact list at the consultation. You can often arrange some of these in Bologna through the public system or privately, then send the results to your coordinator.

Two core procedures are offered. Sleeve gastrectomy reduces stomach volume. It is aimed at weight loss and a quick feeling of fullness, and it suits obesity without marked reflux. Gastric bypass reduces stomach volume and changes the path of food. It aims at weight loss and better metabolism, and it suits obesity with diabetes or reflux. The clinic also carries out Roux-en-Y gastric bypass, mini gastric bypass (MGB) and SADI-S duodeno-ileal bypass. The choice is individual and based on BMI, related conditions and your goals.

Both procedures are done under general anaesthesia, using modern laparoscopic technique through 4 small punctures of 5 to 15 mm. Cosmetic sutures are placed, so once healed the marks become almost invisible.

Not everyone is a candidate. The contraindications are:

- age under 18;

- malignant tumors;

- pregnancy;

- blood clotting disorders;

- severe chronic and mental illness;

- acute illness or flare-ups of chronic disease;

- less than six months since breastfeeding ended.

The Italian criteria also say alcohol, drugs and smoking should be stopped before surgery. If any of these apply to you, tell the coordinator early. This is a medical conversation, not a judgement.

The package covers preparation, the operation and the work of the operating doctor, general anaesthesia, the inpatient stay and meals at the clinic. It also covers a year of support after discharge, with one coordinator from day one and remote progress checks by photos. To reserve a date, you pay a deposit by card at the clinic or online by bank transfer. The terms are fixed in the medical services contract.

Follow-up after the procedure: 12 months of online support and beyond

After discharge you get advice on diet and recovery. You also get scheduled check-ups of your well-being, weight and test results. The clinic team supports you for a year. Because the follow-up is remote, you do not need to plan around Bologna's transport or the working day. A coordinator reminds you about tests and check-ups, answers questions during recovery and books follow-up visits. Progress is checked from photos you send.

Weight change takes time. The final result comes 18–24 months after the procedure. During that period the body becomes stable, weight reaches target values and nutritional status normalises. That is also why doctors advise holding off pregnancy for at least 12–18 months. Pregnancy after bariatric surgery can happen, and it is best planned with the medical team.

Vitamins are essential. The digestive anatomy changes, so the body absorbs certain nutrients less well. Regular vitamin and mineral supplements become a lasting part of life. Ask the coordinator to help you set up a reminder routine. Taking them at the same time each day is simpler than relying on memory.

Weight regain is possible, but it is very unlikely if you follow the doctor's advice. Proper eating, avoiding high-calorie drinks and regular follow-up with the specialists are the key factors. The national data on sedentary behaviour is a useful reminder. If your job or studies keep you at a desk, build short walks into the day. If something changes, such as a new work schedule, a move or an illness, message the coordinator. Do not wait for the next scheduled check-up.

Long-term care also includes your Italian health system contacts. Keep your family doctor informed, and tell any specialist you see that you have had bariatric surgery. Our team can give you a summary of your plan to share. This helps public and private care work together.

Does the Italian national health service pay for bariatric surgery, and is a private online consultation still useful?

A January 2024 article describes procedures needed to reduce the risks of severe obesity, including bariatric surgery, as reimbursable by the SSN. Purely aesthetic surgery is not covered. Coverage and pathways are organised regionally, so ask your regional health service how it works for you. A private online consultation does not replace that process, but it gives you a doctor's assessment and a clear plan at no charge. It also does not commit you to surgery.

Are the Italian BMI criteria the same as the ones used by your doctors?

They are very close. The Italian criteria list an adult with BMI of 40 kg/m2 or higher, or between 35 and 39.9 with one or more obesity-related diseases, after conservative therapy has failed. Our doctors generally consider surgery above 40, or above 35 with serious related conditions. Sometimes they consider it at lower values when non-surgical methods have not worked. The final decision is made during your consultation.

How long might I wait if I choose the public system, and how does that compare with a private route?

English-language reference material on Italian healthcare says waits are usually up to a few months in the big public facilities. It says private facilities contracted with the SSN usually take up to a few weeks. We found no official waiting-list figure, so these are general descriptions. With us, the first assessment is online and free. The date is agreed with you and fixed in the contract before anything is booked.

Is it better to schedule the procedure outside Bologna's hot summer?

The doctor's plan and your health come first. As for the weather, the 1991–2020 July normals show a mean high of 31.4 °C, with records reaching 40 °C. That can make early walks tiring. Winter has a mean January low of -0.5 °C, and November is the wettest and most humid month, at 81% humidity. Neither season is ruled out. Discuss your preferences with your coordinator, who can suggest a date that suits both your recovery and your calendar.

I study at the University of Bologna. Can I fit the recovery around the academic year?

Many people return to office work after 2–4 weeks, and physical work after 1.5 months. The hospital stay is 1–5 days. We do not know your timetable, so share it with your coordinator. Also think about meals on campus, since the diet moves from liquid to soft to solid. The programme is flexible, but plan the first weeks with care.

Can I reach a coordinator on Italian public holidays and Sundays?

Yes. Our coordinators answer 24/7 on WhatsApp and Telegram, including on Sundays and on national holidays such as 1 November, 8 December and 25 and 26 December. The online consultation is free and the first reply usually arrives within 15 minutes. Some laboratories and hospitals may keep different hours on those days, so allow for that when arranging tests.

What should I do if I already have tests from a hospital in Emilia-Romagna?

Send them to the coordinator through WhatsApp or Telegram. The doctor will review them during the consultation and tell you which ones can be used and which need repeating. The usual list before the procedure is blood and urine tests, an ECG, an ultrasound and a gastroscopy. You may need specialist appointments too. Having earlier results can shorten the preparation, but the doctor makes that decision.

How does follow-up work if I am in Bologna and the team is online?

Support lasts 12 months and is organised remotely. One coordinator stays with you from day one, reminds you about tests and check-ups, and books follow-up visits. Progress is checked from photos. The full result comes 18–24 months after the procedure. You can message at any hour if something worries you, and the team will tell you whether it needs urgent attention.
