How obesity surgery works for Bochum residents

All bariatric operations here are laparoscopic, performed through four small punctures of 5 to 15 mm with cosmetic sutures, so the marks become almost invisible after healing. This greatly shortens recovery. For those from Bochum, the choice between sleeve, classic bypass, mini bypass and SADI-S depends on BMI, related diseases and the result you want to reach.

It begins with a surgeon's consultation: medical history, choice of method and a recovery plan. Patients from Bochum then complete a work-up — blood and urine tests, biochemistry, HbA1c, coagulation, chest X-ray, ECG, abdominal and kidney ultrasound, leg vein Doppler and a required gastroscopy — plus consultations with a GP, endocrinologist, psychiatrist, cardiologist and, for women, a gynecologist.

Your investment covers more than surgery. The package includes the full diagnostic programme, the chosen laparoscopic method, inpatient care with food and nursing staff, round-the-clock medical presence and specialists tracking your weight and tests throughout the first year. For people searching for weight loss surgery in Bochum, the first consultation with the surgeon is free and takes about 15 minutes.

Obesity figures in Germany: why the survey and the measurement differ

When Bochum residents look at national obesity statistics, they often find two sets of numbers that seem to contradict each other. The Mikrozensus survey of 2009 recorded adult obesity (BMI 30 and above) at 15.7% of men and 13.8% of women. The measured data of the DEGS study from 2008 to 2011 gave 23.3% of men and 23.9% of women. The gap exists because people report their own weight and height in a survey, and self-reported values tend to be lower than values taken with a scale and a tape measure. For a patient, this means the true picture is usually less comfortable than the questionnaire suggests.

The same lesson applies to your own starting point. Before the consultation we ask for measured values, not estimates: weight in kilograms, height in metres, and ideally a photo of the readings. BMI is body weight divided by height squared, so a few centimetres of error in height can shift the result more than you would expect. A coordinator on WhatsApp or Telegram can help you check the arithmetic in a couple of minutes.

The regional setting is worth a short note. Bochum has 358,880 inhabitants (31 December 2025) and lies inside the Rhine-Ruhr metropolitan area, which has 5,166,484 inhabitants. The national figures above are not specific to the city, and we do not claim a local obesity rate. What the size of this urban region does show is that the question of weight-loss treatment concerns a very large number of people, and that no one needs to feel unusual about asking.

Bariatric surgery is a medical treatment of obesity and the illnesses linked to it. It is not a cosmetic procedure, and the two should not be mixed up. The operation changes how the digestive tract works. It does not reshape the outer contours of the body. Hypertension, type 2 diabetes, sleep apnea, joint disease and fatty liver disease are the conditions the operation can help with, and this is why the German S3 guideline treats the question as a matter of medicine, not appearance.

The same guideline sets the thresholds. Bariatric surgery is indicated from BMI 40 kg/m² even without comorbidities, and from BMI 35 kg/m² when an obesity-related condition is present, once conservative therapy has failed. Our doctor uses the same two thresholds at the free consultation. The service also notes that surgery is sometimes considered at lower values when non-surgical methods have not worked, but that is always an individual decision.

What statutory insurance covers and what it does not

About 92% of the German population is covered by statutory health insurance (GKV). Among the working population in 2024, about 88% were insured through statutory funds and about 11% privately. So most readers from Bochum will have some form of statutory cover, and it is sensible to understand exactly where it stops with regard to weight-loss surgery.

Bariatric surgery is not part of the regular benefit catalogue of the GKV. It can, however, be applied for in individual cases. The conditions are demanding. You must prove that conservative weight-loss methods were exhausted over 6 to 12 months in a multimodal concept, and the Medizinischer Dienst (MD) must assess your case under the "Begutachtungsanleitung Adipositas-Chirurgie". In practice, this means keeping records: notes from diet counselling, exercise programmes, and the dates and results of earlier attempts.

It helps to separate this from cosmetic surgery, where the rules are clear. The statutory insurance does not pay for cosmetic procedures done at the patient's request, and the patient bears the full cost, including lost income. Insurers may also restrict or refuse benefits under § 52 SGB V when a procedure was not medically indicated, and there is no entitlement to continued wage payment (Entgeltfortzahlung) in that case. This is why the medical indication matters so much. A documented BMI, related conditions and a record of earlier treatment turn a request into a clinical case.

We do not process insurance applications on your behalf and cannot predict how an insurer will decide. What we can do is give you a written plan after the free consultation, describing the indication, the proposed method and the reasoning behind it. Many patients use such a document when speaking with their insurer or their family doctor. The final price of the programme is named by the doctor and fixed in the contract before anything is booked, so you can compare it with any decision from your fund.

Waiting times, planning and the pace you set

One figure from the national picture is often quoted: a 2010 Commonwealth Fund survey found that 70% of Germans reported waiting less than one month for elective surgery. It is an older survey and concerns elective operations in general, so it does not describe bariatric waiting lists in any specific region. It does show that, for planned procedures, the German system is not known for very long queues. Bariatric operations covered through the insurer's individual-case route add an extra step, the MD assessment, and that step is where most of the waiting tends to occur.

An online consultation works on a different timescale. You send photos and basic data on WhatsApp or Telegram, the doctor gives an assessment and a price, and you confirm the plan and dates before anything is booked. Coordinators answer round the clock, and the stated reply time is within 15 minutes. If you are waiting for a decision from your insurer, you can still have the consultation and keep the plan in your folder, so you know your options whichever way the decision falls.

For comparison, the national data give a sense of scale for other planned operations. About 400,000 hip and knee joint replacements were performed in Germany in 2016, and the inpatient stay after an uncomplicated hip replacement is about six to eight days in most clinics. In 2014, a gastric bypass was performed in 4,083 cases on obesity patients in Germany, according to Statista as cited. Compared with a joint replacement, a bariatric stay is short: 1 to 5 days under observation, depending on the complexity of the operation and how you feel.

The plan itself has clear steps. After the consultation come tests: blood and urine tests, an ECG, an ultrasound and a gastroscopy, plus visits to the specialists the doctor names. Then comes the operation under general anaesthesia, the inpatient stay, and recovery with check-ups for 12 months. You set the pace of the early steps, because nothing is booked until you confirm.

Two operations, four methods: how the choice is made

The comparison table of the service lists two core procedures: sleeve gastrectomy and gastric bypass. The fuller description adds that the clinic carries out four main types: sleeve gastrectomy, Roux-en-Y gastric bypass, mini gastric bypass (MGB) and SADI-S duodeno-ileal bypass. The choice is individual and rests on BMI, related conditions and your goals. Nobody should pick a method from a forum post.

Sleeve gastrectomy reduces the stomach volume. Its main goal is weight loss and a quick feeling of fullness, and it is usually considered for obesity without marked reflux. Gastric bypass reduces the stomach volume and changes the path of food. Its aims are weight loss and better metabolism, and it is usually considered for obesity combined with diabetes or reflux. In the German material on gastric bypass, the operation is described in the same terms: a smaller stomach and a rerouted food path. The doctor explains in the consultation which of these fits your case, and why.

Most patients ask about scars. The operation uses a laparoscopic technique through 4 small punctures of 5 to 15 mm. Cosmetic sutures are placed, and once healed the marks become almost invisible. Everything about the extent of the operation and the recovery period depends on your anatomy and individual traits, and the doctor confirms it in the consultation.

Some people are not suitable candidates, and the consultation is where that is established. The contraindications are age under 18, malignant tumors, pregnancy, blood clotting disorders, severe chronic and mental illness, acute illness or flare-ups of chronic disease, and less than six months since breastfeeding ended. If you are unsure which group you belong to, message the coordinator before the consultation. Candid answers save time for everyone, and a refusal for medical reasons is part of good care.

Food after the operation, and what Bochum's seasons add to the routine

After the operation, your diet moves in stages from liquid to soft to solid food. The main focus is protein and restricting high-calorie, fatty food. High-calorie drinks deserve a special warning, because the doctor lists avoiding them among the key habits against weight regain. Regular vitamin and mineral supplements become a lasting part of life, because the changed anatomy means the body absorbs some nutrients less well.

We do not have sourced facts about what people in the Ruhr area typically eat, so we will not describe a regional menu. What we can offer is a method. Write down your usual week of meals before the consultation, including drinks, and bring it to the conversation. The doctor and the coordinator then adapt the staged diet to your real routine instead of a generic one. If you have a student or shift-work schedule, say so: Ruhr-Universität Bochum alone has 37,653 students (winter semester 2025/26), and the city has nine universities or branches of universities, so irregular timetables are a common reality here.

The climate shapes how you plan activity. Bochum's mean daily maximum is 5.6 °C in January and 24.7 °C in July, with a January mean minimum of 1.2 °C. There are 195 rainy days a year and annual precipitation is 866 mm, with December the wettest month at 93 mm. Sunshine averages only 1.3 hours a day in December and 1.5 hours in January, against 7.0 hours in July. The service text says swimming is fine after 30 days and other exercise is added gradually as weight drops. If your recovery falls in a wet, dark month, plan indoor options such as walking in covered spaces or home exercises, and leave outdoor routes for drier days.

The terrain is gentle but not entirely flat. The city lies on the flat-hilly Bochumer Landrücken (Ruhrhöhen), at about 100 m above NHN, with the highest point at 196 m and the lowest at 43 m. The Kemnader See, a dammed section of the Ruhr, is one of the larger bodies of water in the area. Whatever route you choose, begin short and add distance as your energy returns. Physical work resumes after about 1.5 months, and office work after 2 to 4 weeks.

Social calendars also matter. Bochum Total, a free open-air festival begun in 1986, is usually held on the first July weekend, and the Christmas market runs through December downtown. These are events where food and drinks are everywhere. We do not tell anyone to avoid them. We suggest deciding in advance what you will eat and drink, so that the plan from the doctor is not tested for the first time in a crowd.

Follow-up online: twelve months, then the long game

The programme includes preparation and support for 12 months. One coordinator is with you from the first day, and progress is checked remotely by photos. After discharge you receive advice on diet and recovery, and scheduled check-ups of your well-being, weight and test results. Because the contact runs through WhatsApp or Telegram, you do not need to arrange time off for every question. Coordinators answer round the clock.

The final result takes 18 to 24 months to develop. That is longer than the first months of rapid change, and it is worth saying clearly, because early enthusiasm can fade. Weight regain after bariatric surgery is possible, though the service text calls it very unlikely when you follow the doctor's advice. The three habits it names are proper eating, avoiding high-calorie drinks and regular follow-up with specialists. The online format makes the last one easier to keep.

A practical way to use the follow-up is to treat it like a diary. Send photos on the agreed dates, write down questions as they come, and note any symptom that worries you. When you move between seasons, for example from the short, grey days of December to the long July evenings of 7.0 hours of sunshine, review the routine again with the coordinator. A habit that suits January in Bochum may not suit July.

Family planning needs attention too. Pregnancy after bariatric surgery can happen, though doctors advise holding off at least 12 to 18 months, while the body becomes stable, weight reaches target values and nutritional status normalizes. If this concerns you or your spouse, raise it at the very first consultation so the plan accounts for it.

Your own family doctor, if you have one, remains important for everyday care. We recommend sharing the plan with them so that your prescriptions and routine blood tests stay consistent with what the clinic advises. The online follow-up complements that relationship and does not replace it.

Does German statutory insurance pay for weight-loss surgery?

Not as a routine benefit. Bariatric surgery is not part of the regular benefit catalogue of the statutory insurance, but it can be applied for in individual cases. The conditions include proof that conservative weight-loss methods were exhausted over 6 to 12 months in a multimodal concept, and an assessment by the Medizinischer Dienst under the "Begutachtungsanleitung Adipositas-Chirurgie". We cannot predict an insurer's decision, but the free consultation gives you a written indication and plan to use in that process.

Why do German obesity statistics show such different numbers?

Because surveys and measurements differ. The Mikrozensus of 2009 reported adult obesity of 15.7% in men and 13.8% in women, while measured DEGS data from 2008 to 2011 gave 23.3% in men and 23.9% in women. People tend to report lower weights than a scale shows. For your own planning, use measured weight and height, not estimates, so the BMI calculated at the consultation is accurate.

Which BMI thresholds does the German guideline use?

The German S3 guideline indicates bariatric surgery from BMI 40 kg/m² even without comorbidities, and from BMI 35 kg/m² with an obesity-related comorbidity, once conservative therapy has failed. These match the thresholds our doctor uses: usually a BMI over 40, or over 35 with related conditions. In some cases lower values are considered when non-surgical methods have not worked. The doctor confirms your situation at the consultation.

Can I have the consultation while my insurer is still deciding?

Yes. The consultation is free, takes place online and does not commit you to surgery. You send photos and data on WhatsApp or Telegram, receive an assessment and a price, and nothing is booked until you confirm the plan and dates. Many patients keep the plan as a reference while the Medizinischer Dienst assessment is pending, since the decision of the insurer and the medical plan are separate matters.

How should I plan exercise during Bochum's wet and dark months?

Use the sourced figures as a guide: December brings 93 mm of precipitation and only 1.3 hours of sunshine a day, and January has a mean maximum of 5.6 °C. Swimming is fine from 30 days after the operation, and other exercise is added gradually as weight drops. Plan indoor options for the wettest weeks and move outdoors on drier days. Start short, and ask the coordinator to adjust the routine at each check-up.

How does the online follow-up work after the operation?

The package includes preparation and support for 12 months with one coordinator from day one. You get diet and recovery advice, scheduled check-ups of well-being, weight and test results, and remote progress checks by photos. Coordinators answer on WhatsApp and Telegram round the clock. Regular follow-up with specialists is one of the three habits that make weight regain very unlikely, along with proper eating and avoiding high-calorie drinks.

Is bariatric surgery treated like cosmetic surgery by German insurers?

No, but the distinction matters. Statutory insurance does not cover cosmetic procedures done at the patient's request, and the patient bears the full cost, including lost income. Bariatric surgery is a medical treatment of obesity and related illnesses, which is why a documented indication such as BMI and comorbidities is so important. The plan from the consultation sets out that indication in writing.
