Closed Rhinoplasty

Closed (endonasal) rhinoplasty reshapes the nose entirely through incisions hidden inside the nostrils, leaving no external cut at all. Recovery tends to be gentler because the skin is not lifted away, and the price starts from $4,200, the same as for the open approach.

What closed rhinoplasty is

In the closed approach every incision stays inside the nostrils. The surgeon frees the skin from the underlying framework without turning it back and reshapes the bones, cartilage and septum working through these hidden openings. Nothing is cut on the outside, so there is no external scar to heal at all.

The technique is also called endonasal rhinoplasty, and it is one of the two approaches our surgeons use; the choice between them depends on what your nose needs.

When the closed technique is the right pick

Working endonasally suits straightforward cases: smoothing a hump, modest reduction, straightening the bony part, correcting a deviation that does not require rebuilding the tip. Because the skin is not lifted, swelling after surgery is usually milder and the early look settles sooner than after an open operation.

The trade-off is visibility: the surgeon reshapes through a narrow field with a limited direct view of the framework. Cases that need detailed tip reconstruction, graft placement or correction of a previous operation are usually better served by the open approach. The surgeon names the approach for your case at the free consultation, together with the plan and the price.

Active infections in the nose, bleeding disorders and conditions that make general anaesthesia unsafe postpone the operation. Smokers are asked to quit ahead of the date to protect healing. There are contraindications, and the doctor decides whether surgery suits you after examination and tests.

Preparation starts before the date: the tests and the anaesthetist's review, a written instruction on which medicines and supplements to pause, and a complete break with smoking, because it slows the healing of the mucosa and the skin. You arrive on the day without make-up, and the agreed plan is re-checked and signed before the anaesthesia.

How the operation goes

Closed rhinoplasty is done under general anaesthesia and takes 1.5 to 3 hours depending on complexity. Afterwards you spend the first 24 hours under observation in the inpatient unit and leave the next day with aftercare instructions and a follow-up schedule. Internal splints may support the septum while the tissues settle.

Recovery after a closed rhinoplasty

Because the skin was never lifted off the framework, the swelling that remains after a closed operation is usually less pronounced than after an open one.

Closed or open: how the surgeon decides

Both approaches reshape the same structures; they differ in access, not in the result they aim for. If your case needs only reshaping that can be done through the nostrils, closed surgery keeps the outside of the nose untouched and recovery lighter. If the tip needs detailed rebuilding or it is a second operation, the open view wins. The decision is made at the consultation, after the surgeon has examined your nose, not by marketing labels.

Bring the questions in writing: whether your tip can really be corrected through the nostrils or the open view would serve better, how the septum will be handled, what the swelling curve looks like for your skin type, and when the tracking photographs are taken. Honest answers here outweigh any gallery of other people's results.

Frequently asked questions

Does closed rhinoplasty leave a scar?

No external scar: every incision is hidden inside the nostrils, so the outside of the nose is untouched.

How is it different from open rhinoplasty?

The open approach adds a small cut across the columella and gives a direct view of the framework; the closed approach works through the nostrils with a limited view but usually causes less swelling.

How much does a closed rhinoplasty cost?

From $4,200 ($6,000 without the discount). The price includes the operation, the day of observation in the clinic and follow-ups, and it is fixed in the contract after the free consultation.

How long does the operation and the stay take?

The operation runs 1.5 to 3 hours under general anaesthesia, and you stay the first 24 hours under observation in the inpatient unit.

When will the swelling go down?

The cast comes off on day 14 and most bruising fades around then; main healing takes about 4 months, and check-ups at 3 and 6 months track the settling shape.

Am I still a candidate if I need tip work?

Detailed tip reconstruction is usually better done open, where the surgeon sees the framework directly. The approach for your nose is decided at the consultation.
