Seven answers to collect in writing before any date is held
Madrid is a city of 3,477,497 people (2025 figure for the city itself), and the wider Community of Madrid counts 7,113,886 (2025) across 179 municipalities. With that many people looking for hair restoration and that many private centres to choose from, the best protection is paperwork rather than impressions. Before a date is reserved, ask for seven things in a single message and keep the replies in one folder.
First, the medical licence of the organisation that provides care. The City Medical Centre clinic holds one, and a patient may ask to see it. Second, the full name and specialty of the person who will carry out the transplant. Third, the credentials of every doctor who will take part: diplomas and certificates may be inspected before payment. Fourth, the contract for paid medical services, read in full and not summarised aloud. Fifth, the method that has been proposed and the reason it was chosen. Sixth, the graft count and the area it is meant to cover. Seventh, a list of what the arrangement includes, and of what it leaves out, with no figures needed at this stage.
Two further points separate a serious answer from a vague one. Ask who will examine you afterwards, and ask what is written down as opposed to what is only said. A reply that swaps the doctor you met for a different name deserves a follow-up.
A practical habit helps here: when a reply arrives by phone or by voice note, ask for the same content in a written message, then read it as a stranger would. The order speaks of cosmetic surgery in general, but it points to a fair question for any clinic: which specialty did this doctor complete, and where can I see it recorded?
The Ilustre Colegio Oficial de Médicos de Madrid, known as ICOMEM, is the professional body for the city's doctors. Its website offers a member finder, the Buscador de Colegiados, and a membership roll. Treat it as a check on a name you already hold. Take the name from the clinic, search it, and see whether what you find matches what you were told.
Spain also maintains national registers run by the Ministry of Health. In the Registro Estatal de Profesionales Sanitarios, citizens search by the name of a professional, a centre or a province and see official qualifications and the centres where the person works. The ministry says the register is still being completed, so the absence of a name does not by itself mean a lack of the right to practise. Use a missing entry as a reason to ask a question, not as a verdict.
Who pays? The same Observatory source places aesthetic interventions outside the benefits basket of the national health system and notes that private centres offering them have multiplied. A cosmetic transplant is therefore settled directly with the clinic, under a contract. If you suspect a medical reason behind the loss, take that to your own doctor first.
Pictures and sentences that let a surgeon answer you properly
Madrid residents begin with a message. The consultation is free and happens online, nothing is booked, and a coordinator replies on WhatsApp or Telegram at any hour, with a stated reply time of 15 minutes. To start, send a phone number and photographs of the thinning places; a doctor looks and says whether a transplant would help.
Make the photographs easy to read: daylight, dry hair, no flash, a plain wall behind you. Cover the top of the head, the front of the scalp, both temples, the back, and the areas from which grafts would be taken. An older picture in which your hair looked fuller shows the doctor what you are comparing against.
Under the pictures, write a short note. When did thinning begin? Has it been steady or sudden? Does it run in the family? What have you tried? Mention illnesses and medicines, and read the caution list in the last section before you hit send. Add one sentence about what you want: a denser front, coverage at the crown, or a beard or eyebrow instead of the scalp.

What the weeks after the operation look like, in order
Match the clinic's method to your own donor zone: FUE uses forceps with no incisions and no stitches, DHI uses a Choi pen with no channels opened beforehand, and a combined protocol extracts by FUE and implants by DHI. Ask the doctor which of the three fits your donor zone and why.
One day, local anaesthesia, home the same evening. After that the clinic describes the road in stages: 0–10 days, 2–4 weeks, 3–4 months, 6–9 months and 12–18 months, with check-ups at 1, 3, 6, 9 and 12 months. No doctor gains by sending you into theatre needlessly, and the clinic says so. A transplant is considered for androgenetic or diffuse alopecia, bald areas and a receding front of the scalp.
The caution list is longer, and it reads like a medical history: cancer, epilepsy, diabetes mellitus, serious heart conditions, a pacemaker, pulmonary embolism, hepatitis B or C, AIDS or HIV and intolerance to anaesthetics on the health side; trichotillomania and body dysmorphic disorder on the psychological side; being under 18, insufficient hair density, genetic traits of hair growth, a high frontal line, patchy alopecia, damage from a burn or injury, and shedding after an illness or from hormonal imbalance.
If anything on the list applies to you, do not wait for the doctor to find it. Put it in the first message, and ask the coordinator to flag it for the surgeon who reviews your photographs. Honest disclosure at this stage saves a longer conversation later and keeps the plan realistic.
