FUE vs DHI: How the Two Hair Transplant Methods Differ and Which One Suits You

FUE and DHI in one sentence each

FUE (follicular unit extraction) removes grafts one by one from the donor area, the surgeon then opens small channels in the thinning area and places the grafts into them. DHI (direct hair implantation) uses the same kind of extraction, but implants each graft with a pen-shaped Choi implanter without making the channels in advance.

So the real difference between FUE and DHI is not how grafts are taken, but how they are put back. That single step changes the handling of the grafts, the time the operation takes, the kind of areas each method suits and the price.

How FUE works

FUE is the classic technique. After local anaesthesia, grafts are taken individually from the back of the head or the sides, the donor area. There are no incisions or stitches, which is why FUE replaced the older strip method for most patients. The NHS describes the strip method, FUT, as removing a strip of skin that leaves a single scar, while FUE leaves many tiny scars that are less noticeable.

Once the grafts are collected, channels are opened across the recipient area and the grafts are placed into them. Because the channels are prepared in one stage and filled in another, FUE works efficiently over larger zones such as the crown or the whole top of the head.

How DHI works

In DHI each graft is loaded into a Choi implanter and inserted directly into the scalp. The pen creates the opening and places the graft in one movement, which gives the surgeon fine control over the angle, direction and depth of every hair.

That control is why DHI is often chosen for the hairline, the temples and for adding density between existing hairs. The trade-off is that implanting graft by graft with a pen is slower and more demanding, which is reflected in the price.

Which is better, FUE or DHI?

Neither method is better for everyone. The right choice depends on where you are losing hair, how much of the existing hair is still there, the quality of your donor area and what you want the result to look like.

• FUE often makes sense when a large area needs coverage, for example a receding hairline together with a thinning crown.
• DHI often makes sense when the priority is a natural, precisely designed hairline, or when grafts must go between hairs that are still growing.
• A combination is used when both apply: grafts are extracted by FUE and implanted by DHI.

Be wary of any clinic that recommends one method before seeing your scalp. The graft count and the method should come out of the examination, not the other way round. Recovery time: FUE vs DHI

Recovery is much the same for both. Extraction is identical, so the donor area heals in the same way, and in both cases you go home on the day of the procedure. What about FUT and sapphire FUE?

Is DHI better than FUE?

Not in general. DHI gives more control over the angle and direction of each graft, which helps with hairlines and adding density between existing hair. FUE is efficient for larger areas. The method should be chosen after an examination of your hair loss and donor area, not in advance.
