Surgery is prescribed only when clear criteria are met
The main way to assess the degree of obesity is BMI. It is body weight (kg) divided by height squared (m²). Generally, surgery is justified with BMI above 40, or above 35 combined with serious related conditions. Lower values may sometimes be considered for surgery too, when non-surgical measures have failed.
Many try all kinds of diets and exercise for years, yet the body defends itself — metabolism drops, hunger grows and lost weight returns. Once it is obvious that medication and a changed diet alone cannot fundamentally fix the situation, bariatric surgery becomes the only effective path.

Sleeve gastrectomy
The operation involves removing most of the stomach and turning it into a narrow tube. As a result portions shrink a lot and the hunger hormone ghrelin falls, so the patient reaches fullness sooner and eats less.
Effectiveness: the method is used for a BMI of 35 to 45; average loss is 60–70% of excess weight.
Specifics: less durable results than bypass operations. Without sticking to the diet, especially avoiding high-calorie drinks and liquid food, the weight may return.

Roux-en-Y gastric bypass
At the top of the stomach the surgeon makes a small 50–70 ml pouch, linking it directly to the small intestine. Intake of food is restricted and calorie absorption falls, since food passes through less of the gut.
Effectiveness: considered the “gold standard” when BMI exceeds 45–50 with type 2 diabetes. Patients lose 75–85% of excess weight.
Specifics: the result is more durable than with other bariatric techniques. Diabetes, hypertension and similar related conditions often enter remission. The changed digestive anatomy means lifelong vitamin and mineral supplements.

Mini gastric bypass (MGB)
This procedure improves on the classic bypass. Instead of the standard approach, the surgeon creates a single connection (anastomosis) between the small stomach and the small intestine. A simpler technique means an easier experience for the patient and a shorter recovery.
Effectiveness: matches the classic Roux-en-Y bypass — patients' excess weight loss is up to 75–80%.
Specifics: shorter surgery, quicker recovery. It is technically reversible, yet bile reflux is a risk because of the single anastomosis.

Single anastomosis duodeno-ileal bypass (SADI-S)
SADI-S is a modern combination technique that draws on the benefits of both sleeve gastrectomy and intestinal bypass. In two stages, the stomach is first converted into a narrow tube and the small intestine is then rerouted to exclude its greater part from active digestion.
Effectiveness: among the most powerful bariatric operations — excess weight loss reaches 80–90% with a lasting result.
Specifics: removal of the ghrelin-producing zone shrinks the stomach and appetite, and routing food past part of the intestine curbs calorie absorption. Appropriate for super obesity (BMI over 50), severe type 2 diabetes, or as a second stage after a sleeve that failed.

At City Medical Centre obesity is treated with a comprehensive approach. Once examined, each patient receives a personal lab and instrumental diagnostic program aimed at finding every disorder in the body.

A method matched to your goal
Our doctors decide on the surgical method case by case. Before choosing, they evaluate your health, weight history and related conditions in order to offer the surgical treatment that fits best.

Support that stays in touch
The City Medical Centre team stays with you after surgery. Throughout the first year, specialists track your weight loss, monitor your tests and answer all questions about diet and recovery.

Fast recovery
Bariatric operations at City Medical Centre are laparoscopic, which greatly shortens recovery. Most patients are up and moving on day one and return to normal life within a week.

Full diagnostics on site
No need to travel between clinics. From our own lab to ultrasound and gastroscopy, the centre has all you need for your work-up. Preparation is fast, without queues, in one building.
Recovery is mostly down to the individual; still, minimally invasive laparoscopic methods keep rehabilitation as short as possible.

Duration
Observation at the clinic lasts 1 to 5 days, based on how extensive the surgery is and on related conditions.
Activity
Within the first hours after surgery you may sit up, stand and drink fluids.
Wound care
Absorbable sutures are used; they dissolve on their own.

Work
You can go back to work 7–14 days after surgery, depending on how physical the job is.
Restrictions
No sauna, no bathhouse and no intense physical exercise for the first weeks.
Diet
A staged diet is set for 1–2 months — from liquid to solid food — so the stomach can adapt.

Pace
When they follow specialist advice, patients lose an average of 10% of excess weight each month.
Stabilization
The final outcome arrives within 1.5–2 years. By this time the patient has lasting eating habits that stop the weight returning.
Important! Your recovery is overseen by City Medical Centre specialists, who offer psychological and medical support throughout each stage of weight loss.

Bariatric vs plastic surgery: what's the difference?
Though often confused, the two have fundamentally different goals. Bariatric surgery is a medical treatment of obesity and related illnesses; it changes the functioning of the digestive tract to address the causes of excess weight and metabolism. Plastic surgery reshapes the outer contours of the body and neither treats obesity nor affects metabolism.
How soon can I return to work and normal life?
Will there be visible scars?
We rely on modern laparoscopic technique. The operation is performed through 4 small punctures of 5 to 15 mm. Cosmetic sutures are used, so after healing the marks are barely visible.
What diet do I need to follow?
Your diet changes in stages: from liquid food to soft, then to solid. The focus is on protein and on limiting high-calorie, fatty food.
What types of bariatric surgery are there?
City Medical Centre performs four main types of bariatric surgery: sleeve gastrectomy, Roux-en-Y gastric bypass, mini gastric bypass (MGB) and SADI-S duodeno-ileal bypass. Each method is chosen individually based on BMI, related conditions and the patient's goals.
Can I plan a pregnancy after surgery?
Pregnancy after bariatric surgery can happen, though doctors advise holding off at least 12–18 months. In this period the body becomes stable, weight reaches target values and nutritional status normalizes.
Do I need to take vitamins after surgery?
Yes, bariatric surgery makes vitamins a must. Since digestive anatomy is changed, absorption of some nutrients drops, and regular vitamin and mineral supplements become permanent in the patient's life.
Can the weight come back?
Although weight regain after bariatric surgery is possible, it is very unlikely if you follow the doctor's advice. Proper eating, avoiding high-calorie drinks and regular follow-up with City Medical Centre specialists are key.

Weight that began in childhood, a family history of obesity, shortness of breath on stairs, avoiding photos — these are common reasons people in Antwerp start thinking about surgery. A consultation is warranted not only by a high BMI but also by related conditions. The surgeon reviews your health and weight history and offers the most suitable of the four surgical methods.
It begins with a surgeon's consultation: medical history, choice of method and a recovery plan. Patients from Antwerp then complete a work-up — blood and urine tests, biochemistry, HbA1c, coagulation, chest X-ray, ECG, abdominal and kidney ultrasound, leg vein Doppler and a required gastroscopy — plus consultations with a GP, endocrinologist, psychiatrist, cardiologist and, for women, a gynecologist.
Before spending on another diet, get a professional assessment. A short no-cost talk with the surgeon, about a quarter of an hour, shows whether your BMI and related conditions justify surgery. If they do, one package takes you through tests, the operation, care in the ward and follow-up, with the weight loss result set in a contract. Residents of Antwerp can apply online.
